Subject: Added `lists_without_preceding_blankline` extension.

* Added `Ext_lists_without_preceding_blankline` to
  `Extension` in `Options`.  Added this option to
  `githubMarkdownExtensions`.
* Made markdown reader sensitive to this.
* Closes #972.
